Department of Public Health (DPH) , Communicable Disease Section is recruiting dedicated
Public Health Nurses II (PHN II) to perform complex case management of individuals with openings in the Congenital Syphilis Prevention Program and Tuberculosis (TB) Program. These positions involve coordinating care, developing treatment plans, and providing consultation on medical, dental, and behavioral health assessments.
PHN II nurses will investigate and manage cases of syphilis and congenital syphilis, ensure compliance with infection control standards, and collaborate with healthcare professionals and public agencies. Responsibilities include disease intervention, partner services, patient follow-up, and data reporting to the California Department of Public Health.
For TB management, duties include developing and monitoring treatment plans, ensuring adherence through direct observation therapy (DOT), conducting quality assurance reviews, and coordinating care among providers and community resources.

Summary of Benefits Memorandum of Understanding (MOU) The County also offers an alternative Modified Benefits Option (MBO) that provides a wage differential. CONDITIONS OF EMPLOYMENT Travel/Driver License: A valid California Class C Driver License is required to carry out the essential job-related functions of this position. Mileage reimbursement and proof of automobile liability insurance must be maintained.

Public Health Nurse Certification: Incumbents must possess and maintain an active Public Health Nurse Certification.
Telework Program: The County allows for the use of telework to qualifying County employees where business needs permit to efficiently serve the community and maintain employee productivity during public health emergencies. Candidates, upon successful completion of a probationary period, may potentially have the option to telework based on their assignment.
Minimum Requirements Licensure/Certification: Applicants
must possess and maintain
all of the following items:
- A valid Registered Nurse (RN) license issued by the California Board of Registered Nursing (CA BRN),
- A valid Public Health Nurse (PHN) certificate issued by the CA BRN,
- A valid Healthcare Provider Basic Life Support (BLS) certificate.
-AND- Experience: Applicants
must meet
one of the following experience options:
- Option 1: Three (3) months of full-time equivalent experience as a licensed Public Health Nurse.
-OR- - Option 2: Three (3) months of full-time equivalent experience in home health or home visiting as a licensed Registered Nurse.
